Appointment Costs
Self Pay (Non-Insurance)

  • Self Pay/Cash Pay: The appointment is charged based on the amount of time spent with the medical provider.
  • $250 per hour with the exception of the ENCORE program.
  • $300 per hour for ENCORE program appointments. 
  • Initial appointments are typically 60-75 mins. 
  • Follow up appointments are typically 30 mins-75 mins. 
  •  If you go over/under the scheduled appointment time we prorate those fees accordingly.
*Appointment costs do not include lab or treatment costs. Many lab fees are billable through your insurance.  Please contact your insurance company to know the details of your lab coverage prior to your initial appointment so we know what your most affordable lab options are. 

Insurance Coverage
We are in-network with the plans shown below, to cover the appointment fees.
For out-of-network insurance carriers, we can provide you with a Superbill to submit to your insurance company, upon request. Some insurance plans, especially PPO's will provide you with reimbursement for your appointment payment.  
Many lab fees are billable to your insurance plan, including plans that we are not in network with. We recommend that you contact your insurance company to know the details of your lab coverage prior to your appointment.  We will then be in a better position to discuss and choose lab options that may be billable to your insurance. 

Co-pay collected at time of service. We will bill United Healthcare for you.  


Co-pay collected at time of service. We will bill Rocky Mountain Health Plans for you.  
Full appointment fees (cash pay fees) are due at time of service. We provide you with a super-bill that you submit to Liberty and Liberty directly reimburses you based on your plan.
